Key Responsibilities\n \n Conduct detailed psychosocial assessments to understand the emotional, mental, and social needs of senior clients, ensuring a thorough exploration of their circumstances. \n Develop individualized care plans tailored to each client's unique preferences and goals, collaborating closely with clients and their families to ensure effective implementation. \n Provide direct counseling and emotional support, assisting clients and their families in navigating significant life transitions, such as end-of-life decisions or moving into assisted living. \n Advocate for client rights and access to local community resources, ensuring seniors receive adequate support and services that align with their needs. \n Regularly engage in client visits via telehealth or in-person where necessary to monitor their well-being, assess any concerns about their living conditions, and follow up on care plans. \n Facilitate communication and resolve any family conflicts that arise during client care, thereby promoting a peaceful support structure for the client. \n Educate clients and families about available community resources, support networks, and programs that can enhance their quality of life. \n Maintain meticulous documentation of client interactions, assessments, and progress notes utilizing electronic health record (EHR) systems, ensuring compliance with all relevant regulations. \n Stay informed on best practices in social work, hospice care policies, and relevant regulations to provide informed and compliant support. \n Patient Population / Client Focus\n In this role, you will primarily focus on senior clients within a hospice setting, addressing their multifaceted psychosocial needs. You will work closely with clients facing terminal illnesses and their familial support networks, helping them cope with emotional distress and making well-informed decisions regarding care options. Your empathetic approach will be vital in fostering a sense of peace and acceptance during a sensitive time.
