Advanced Practice Provider (Delaware County) Delaware County, PA About Us Ennoble Care is a mobile primary care, palliative care, and hospice service provider with patients in New York, New Jersey, Maryland, DC, Virginia, Oklahoma, Kansas, Pennsylvania, Texas, Florida, and Georgia. Our clinicians travel to patients’ homes, providing a continuum of care for those with chronic conditions and limited mobility. We offer programs such as remote patient monitoring, behavioral health management, and chronic care management to ensure our patients receive high‑quality, trusted care. Purpose: We are looking for an experienced Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to deliver primary and palliative care to homebound patients in Delaware County, PA. The role requires applying medical techniques under physician supervision and managing a diverse panel of patients with chronic diseases. What You'll Do: Provide high‑quality care to primary and palliative patients in and around Delaware County, PA. Assess patients through physical examinations, medical history reviews, and develop optimal care plans. Communicate effectively with office staff and clinical operations across internal platforms. Deliver patient‑centered care to a dedicated panel of patients. Develop impressions, diagnoses, and treatment plans. Build trust and consult with patients, families, and caregivers during visits. Order diagnostic testing as medically necessary. Prescribe medications, supplies, and appropriate interventions. What You'll Need: Board Certified with a valid NP or PA certification in Pennsylvania. Strong clinical skills to assess, diagnose, and treat with confidence. Positive demeanor and strong people skills. Ability to problem‑solve independently. 1–2 years of primary‑care experience with geriatric patients (hospice experience highly preferred but not required).
