Registered Nurse – MDS Coordinator

In this critical role, you'll be responsible for delivering expert nursing care to residents, completing comprehensive MDS assessments, developing and presenting care plans, and coordinating discharge and treatment strategies. Your clinical expertise will ensure the delivery of high-quality, patient-centered care in a fast-paced, team-oriented setting.

Required Skills:

• Minimum of 3 years of recent experience as a Registered Nurse, with current licensure in North Carolina or a Compact state
• Proven proficiency in MDS assessments and completion, including baseline care plans and recertifications
• Strong knowledge of long-term acute care and skilled nursing processes
• Experience with CMS regulations, CPT billing, ICD-10 coding, and Medicare/Medicaid guidelines
• Excellent clinical documentation and care coordination skills
• Ability to determine medical necessity and perform quality improvement reviews

Nice to Have Skills:

• Experience with DRG, HIPAA, NCQA, OBRA, and RAPS requirements
• Prior LTAC experience
• Certifications such as RAC-CT, with knowledge of regulatory standards

Preferred Education and Experience:

• BSN degree
• At least 3 years of hands-on, recent nursing experience in a similar setting
• Experience with post-surgical and wound care patients, including pressure ulcers

Additional Requirements:

• Current BLS certification within the last 3 years
• Willingness to work day shifts, 8:30 am to 5:00 pm, with no on-site call or float requirements
• Ability to work in a fast-paced environment with tight deadlines
• Flexibility to work up to 13 weeks with potential for extension
• Candidates must be open to floating within or near the facility as needed
• No RTO during the first week; RTO requests must be pre-approved and no additional RTO permitted after signing
