Home Health Aides, CNAs, LPNs, and RNs deliver compassionate, individualized care to patients in their homes, ensuring comfort, safety, and well-being. These professionals operate within a multidisciplinary team, reporting to nurse supervisors and care managers, and provide essential clinical and supportive services, including monitoring vital signs, assisting with medication, and maintaining detailed patient records. The role requires regular travel within assigned local areas and offers flexible scheduling, ongoing training, and opportunities for career advancement.

Responsibilities

• Provide patient care and assist with personal hygiene
• Monitor and record vital signs and patient health status
• Support patients with daily living activities
• Assist with medication administration as directed
• Maintain accurate documentation and record keeping
• Ensure patient safety and comply with healthcare regulations
• Communicate effectively with patients and family members
• Travel regularly between patient homes within assigned local area

Required Qualifications

• Minimum 1 year of experience in home health care
• High school diploma or equivalent

Preferred Qualifications

• State nursing license as LPN or RN
