Overview Elderwood at Lancaster is seeking a compassionate and dedicated Physical Therapist to join our skilled nursing facility team. Our Physical Therapy professionals play a vital role in ensuring the health, safety, and well‑being of our residents by providing high‑quality, individualized therapy services that support recovery, mobility, and independence. Position Overview Full‑Time: $33.00 – $44.86 per hour. Restore Independence. Strengthen Daily Living. Empower Every Resident. Responsibilities Provide comprehensive physical therapy evaluations and treatments for residents in a skilled nursing setting. Develop and implement individualized treatment plans based on resident needs and physician recommendations. Administer skilled physical therapy interventions to improve mobility, strength, balance, and overall function. Monitor resident progress and adjust treatment plans as appropriate. Collaborate with interdisciplinary team members to ensure coordinated and effective care. Maintain accurate and timely clinical documentation in accordance with regulatory standards. Ensure the highest degree of quality care and resident safety is maintained at all times. Initiate skilled physical therapy treatments in accordance with physician’s orders; maintain required contact with physician. Maintain written evaluations, progress notes and discharge summaries for individuals in the physical therapy program. Confere with other health care staff regarding special needs of individuals to enhance rehabilitation or maintenance of function. Provide input for meetings and attend during absences of the Director Physical Therapy. Maintain statistical records of individuals’ treatments. Assist with in‑service training programs upon request. Record and update individual’s plan of care in the Inter‑disciplinary Kardex. Submit requisition for supplies needed in the treatment to Director Physical Therapy. Assume all duties of the Director Physical Therapy during absences, including supervision of Physical Therapy Aides and responsibility for all necessary documentation. Qualifications Current State Physical Therapy License required. Strong clinical assessment and treatment planning skills. Ability to work collaboratively in a multidisciplinary team environment. Commitment to providing compassionate, resident‑centered care. Graduation from an approved school of physical therapy required. State license to practice physical therapy required. Two years’ experience in physical therapy preferred. Ability to direct others as necessary.
