Chronic Care Management (CCM) Specialist

Fort Myers, FL (On-Site Preferred | Remote Possible for Top Candidates)
$17-$20/hr + Benefits + PTO
Full-Time | Monday-Friday | No Weekends

✅ What You'll Do
Manage patients enrolled in CCM programs
Conduct patient outreach and follow-up calls
Develop and update personalized care plans
Coordinate care with providers and caregivers
Document all patient interactions in eClinicalWorks (eCW)
Educate and support patients in achieving their health goals

What We're Looking For
✔ CCM experience required
✔ Experience with eClinicalWorks (eCW)
✔ Strong communication and organizational skills
✔ Detail-oriented and patient-focused
✔ Comfortable with phone-based patient engagement
