Role Description The Home Health Nurse provides patient-centered nursing care in clients’ homes within the Columbus, Ohio Metropolitan Area. This part-time hybrid role involves traveling to patient residences for in-person visits while completing documentation, care coordination, and follow-up tasks from home when appropriate. Day-to-day responsibilities include assessing patient conditions, administering medications and treatments, monitoring vital signs, and educating patients and caregivers on care plans and health management. The nurse collaborates with physicians, therapists, and other care team members to support continuity of care, updates care plans based on patient progress, and ensures compliance with agency policies and regulatory standards. The role requires accurate documentation, timely communication, and a commitment to safe, compassionate, and culturally sensitive care. Earn up to 400-$500 per day Qualifications Clinical nursing skills, including Nursing and Medicine knowledge relevant to home health practice. Ability to travel to and work in the eastern Columbus area and neighboring suburbs. Experience in Home Care settings, with the ability to deliver services safely in patients’ homes. Strong Patient Care skills, focusing on holistic, person-centered, and culturally respectful care. Active and unrestricted nursing license (LPN/RN) in the state of Ohio. Prior experience in home health, community nursing, or related clinical areas is a plus. Ability to work independently, manage time effectively, and prioritize multiple patient visits. Reliable transportation for travel within the Columbus, Ohio Metropolitan Area. Strong communication and documentation skills, including use of electronic health records. CPR certification and knowledge of basic life support procedures .
