The position assumes dual accountability for Palliative Care and Hospice referrals. Additional Job Duties : Provides palliative care and hospice services based upon patient assessment, including direct patient care. Uses nursing assessment skills to determine the presence of symptoms and response to interventions. Assessments include the full spectrum of patient needs (physical, psychological/social, functional, financial, resource and spiritual). Serves as a consultant to discuss goals of care with the patient/family and communicates those goals to the appropriate care team. Reviews referrals and delegates to the appropriate hospice/palliative care staff based on patient needs and staff availability. Performs hospice consult reviews and coordinates a clinical assessment, determines if the patient is appropriate for hospice, and discusses hospice philosophy, criteria, and site of care with the patient/family. Works as hospice RN with hospital hospice patients, including development of the hospice plan of care and rounding as needed. Acts as a clinical resource for all members of the hospice/palliative team. Maintains knowledge of all services available within the Home and Transitions Service Line. Completes comprehensive and accurate documentation in a timely manner that clearly reflects interventions performed. Collaborates as a member of an interdisciplinary team. Assists with orientation and mentoring of new team members as appropriate. Serves as a resource person and professional role model. Qualifications Associate Degree in Nursing State of New Mexico or Compact State Nursing License BLS certification REQUIRED at time of hire ACLS PALS 1 year of hospice experience required 1 year of acute care experience strongly preferred Current driver's license and auto insurance Basic typing competency We\'re All About Well-being, Starting With Yours.
