RN Care Manager – $3,000.00 Sign-On Bonus

Location: Grand Rapids, Michigan
Schedule: Full-Time, Day Shift - No Nights, Weekends or Holidays
Salary: $72K - $91K/year, based on experience

About the Position:
Grand Valley Medical Specialists is looking for an RN Care Manager to join our team. If you enjoy working with patients over time, helping them understand and manage their health, and coordinating care with providers and other healthcare professionals, this may be a great fit.

This is a clinic-based nursing position for an RN who is ready to step away from bedside nursing while continuing to use their clinical knowledge and make a difference for patients.

You'll work closely with our physicians, advanced practice providers, patients, families, and outside healthcare teams. Much of the role is focused on helping patients with chronic conditions stay on track, coordinating care after a hospital stay, and making sure patients understand what they need to do next.

Grand Valley Medical Specialists is a physician-owned private practice that has been serving the Grand Rapids community since the 1930s. Our team includes 15 physicians and 5 advanced practice providers in internal medicine, pediatrics, and family medicine.

What You'll Do:

• Educate and support patients managing chronic conditions such as diabetes, hypertension, and COPD
• Work with patients to develop care plans and help them take an active role in managing their health
• Follow up with patients after hospital or other facility discharges and help coordinate their transition back to the practice
• Review discharge instructions, arrange follow-up appointments, and identify concerns that may need attention
• Coordinate referrals, home health services, specialty care, and other services
• Communicate with patients, families, providers, specialists, and outside care teams
• Assist patients with medication prior authorizations and patient assistance programs
• Connect patients with community resources and other services that may help them manage their health
• Work with the care team on quality improvement and value-based care initiatives

What We're Looking For:

• Current RN license in the State of Michigan
• Experience in care management, outpatient nursing, case management, or a related area preferred
• Primary care or internal medicine experience preferred
• Strong communication, critical-thinking, and organizational skills
• Comfortable working with electronic medical records
• Able to work independently while staying connected with the rest of the care team
• Familiarity with community resources and patient support programs is helpful
• Experience with value-based care programs such as ACO, PCMH, or HEDIS is a plus

• $72,000–$91,000/year based on experience
