Nurse Practitioner Post-Acute Care Opportunity

We are seeking a compassionate and clinically confident Nurse Practitioner to join a collaborative healthcare team providing high-quality, patient-centered care within post-acute care facilities.

This full-time opportunity offers a predictable Monday-Friday daytime schedule, strong clinical support, and the ability to make a meaningful impact caring for medically complex patients.

The ideal candidate enjoys building relationships with patients, families, nursing teams, and facility leadership while helping improve outcomes and reduce avoidable hospitalizations.

Responsibilities:

• Conduct daily patient rounds within assigned post-acute care facilities
• Perform comprehensive assessments and physical examinations
• Diagnose and manage acute and chronic medical conditions
• Develop and update individualized treatment plans
• Review laboratory results, imaging, and diagnostic testing
• Prescribe medications and treatments within scope of practice
• Collaborate with physicians, nursing teams, therapists, and care managers
• Participate in interdisciplinary care planning
• Maintain accurate electronic medical records
• Support quality improvement initiatives and patient outcome goals
