Position: RN Case Manager

Shift: Full-Time

Weekend Coverage: 5 hours for one Saturday each month; take 5 hours off on one day of the week

Job Summary:

Under general supervision, the RN Case Manager is responsible for the development, implementation, and evaluation of the Case Management Plan. This role involves a highly collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet clients' and patients' health and human service needs. The RN Case Manager plays a critical role in advocating for, communicating with, and collaborating among interdisciplinary team members. Effective resource management and the promotion of high-quality patient care are essential in this position, along with adherence to all regulations and policies relevant to billing and service delivery.

Qualifications:

• Bachelor's Degree in Nursing (Master's degree preferred)
• At least three (3) years of clinical experience in a hospital setting
• At least five (5) years of case management experience in a hospital setting
• Certification in case management (preferred) or willingness to obtain within two (2) years of employment
• Current unrestricted Registered Nurse license in the State of Colorado (required)
• Current Basic Life Support (BLS) certification

Essential Functions:

• Maintain and enhance the case management system through ongoing collaboration with patients, physicians, and other healthcare providers to achieve designated clinical, operational, and financial outcomes for patient populations.
• Document Case Management plans clearly and concisely.
• Assist physicians, patients, families, staff, and payers in delivering appropriate, effective, and cost-efficient care for patient populations.
• Participate actively in departmental and interdepartmental meetings related to Case Management and Social Services functions.
• Collaborate with payers and review organizations to ensure optimal utilization of patient care resources.
• Maintain accurate documentation of all contacts and information within the patient's case management process.
• Educate physicians and staff on appropriate levels of care and utilization issues.
• Stay current on Case Management, Social Services, discharge planning, and nursing issues through continued education.
• Propose and implement new methods and techniques to enhance case management processes.
• Engage in professional development activities to continue professional growth and contribute to the field.
