Job Description & Requirements Specialty: Physical Therapist Discipline: Therapy Duration: 13 weeks 40 hours per week Shift: 9 hours, days Employment Type: Travel Travel Physical Therapist – Home Health Grand Junction,Colorado ⏳ Contract: 13 weeks Shift: Monday–Friday | 8:00 AM – 5:00 PM Schedule: 5 days/week ⏱️ Guaranteed Hours: 40 hours/week Mileage Reimbursement: $0.76/mile Position Overview Seeking a Physical Therapist (PT) to provide skilled, individualized in-home physical therapy services to patients in the Grand Junction area. The PT will promote mobility, safety, recovery, and functional independence through evidence-based care. Responsibilities Perform home health PT evaluations, SOC visits, reassessments, recertifications, and discharge visits Develop individualized plans of care based on patient needs and physician orders Provide gait, balance, mobility, and therapeutic exercise training Implement fall prevention and safety strategies Educate patients and caregivers on treatment plans and home exercise programs Monitor patient progress and communicate changes to the care team Collaborate with RNs, PTAs, OTs, STs, and other clinical staff Provide clinical oversight to PTAs when applicable and permitted Complete accurate and timely documentation in Homecare Homebase (HCHB) Follow Medicare, payer, HIPAA, and regulatory requirements Coordinate patient care and visit schedules with the interdisciplinary team Requirements ✅ Active, unrestricted Colorado Physical Therapist License ✅ Graduate of an accredited Physical Therapy program ✅ Valid driver's license and reliable transportation ✅ Ability to practice independently in a home-based environment ✅ Proficiency in or willingness to learn HCHB ✅ Home Health experience preferred ✅ Medicare/home health documentation experience preferred ✅ Experience with geriatric and medically complex patients preferred Interested?
