Bilingual CCBHC Case Manager - Recovery & Community Support

Care New England

An established health organization is seeking a CCBH Case Manager to support clients in achieving independence and manage their psychiatric and substance use disorder symptoms. This role includes conducting assessments, developing care plans, and connecting clients to essential community resources. Candidates should possess an Associate's Degree, preferably in Human Services, and have strong knowledge of community systems.
