Activity Director

The Activity Director is responsible for planning, developing, coordinating, and evaluating a comprehensive activity program that meets the social, emotional, physical, and cognitive needs of residents in accordance with federal, state of Wisconsin, and facility regulations. The program is designed to enhance residents’ quality of life and promote resident rights, dignity, and independence. Previous experience in LTC is ideal, but willing to train. Essential Duties and Responsibilities Develop, implement, and oversee a resident-centered activity program that meets CMS and Wisconsin DHS requirements. Complete activity assessments and contribute to interdisciplinary care plans based on residents’ interests, abilities, and needs. Plan and facilitate individual, group, and one-on-one activities, including social, recreational, educational, cultural, and spiritual programs. Ensure activities accommodate residents with cognitive, physical, and sensory impairments. Maintain accurate documentation in residents’ medical records, including assessments, care plans, progress notes, and attendance records. Coordinate special events, holiday celebrations, and community outings as appropriate. Recruit, train, supervise, and schedule activity staff and volunteers. Collaborate with nursing, therapy, social services, dietary, and other departments to support resident well-being. Manage the activities budget, supplies, and equipment. Ensure compliance with infection control, safety, and resident rights policies. Participate in care plan meetings, quality assurance activities, and surveys as needed. Serve as a positive representative of the facility to residents, families, and the community. Qualifications High school diploma or equivalent required; associate or bachelor’s degree in recreation therapy, social services, or a related field preferred. Activity Director Certification (ADC) through NCCAP or state-recognized certification required or obtained within a specified timeframe , per facility and Wisconsin regulations. Minimum of one year experience in a long-term care or healthcare setting preferred. Strong interpersonal, organizational, and communication skills.
