Bilingual Registered Nurse Care Coordinator

Martin Luther King, Jr. Community Hospital seeks an RN Care Manager for our Emergency Room to coordinate patient care from arrival through safe discharge. In this role, you will perform thorough assessments, develop and update care plans, and collaborate closely with physicians, social workers, and the interdisciplinary team to ensure timely, appropriate services and smooth transitions of care. You will educate patients and families, advocate for health equity, and connect individuals with community resources that support recovery and long-term wellness.

The RN Care Manager monitors utilization, quality, and safety indicators while maintaining accurate documentation in the EHR. You will participate in huddles, case conferences, and performance improvement initiatives focused on emergency throughput and patient outcomes.
