This position is ideal for a mental health clinician, therapist, social worker, or behavioral health professional who is passionate about providing person-centered, trauma-informed care and supporting clients in community-based settings. What You'll Do Provide Comprehensive Care: Conduct person-centered, culturally responsive, trauma-informed clinical evaluations, including assessment of mental status, risk factors, strengths, needs, and client preferences. Support Clinical Assessment: Gather relevant clinical and historical information, assess client functioning and safety, determine appropriate service needs, and formulate diagnostic impressions using the Diagnostic and Statistical Manual of Mental Disorders (DSM). Develop Individualized Treatment Plans: Partner with clients and, when appropriate, their families or significant supports to develop individualized treatment plans focused on meaningful goals, client preferences, and medically necessary services. Provide Therapy & Mental Health Services: Deliver outpatient therapy, mental health counseling, crisis intervention, case management, Community Support Services, treatment planning, and other behavioral health services consistent with your qualifications, licensure, and applicable standards. Use Evidence-Based Practices: Utilize evidence-based, trauma-informed, and person-centered therapeutic interventions that are appropriate to the client and permitted by applicable regulations and funding requirements. Respond with Care: Address client clinical and behavioral health needs in a timely and responsive manner while maintaining a strong focus on safety, engagement and positive outcomes. Collaborate for Better Outcomes: Work closely with clients, families, treatment team members, external care providers, schools, community agencies, and other service providers to coordinate care and support progress toward treatment goals. Document with Purpose: Maintain accurate, timely, and compliant clinical documentation, including assessments, treatment plans, progress notes, billing documentation, and other required records. Practice with Integrity: Maintain confidentiality, follow ethical standards, practice within the scope of your qualifications and Illinois licensure when applicable, and comply with Family Solutions policies, accreditation requirements, and applicable regulations. Deliver Consistent Services: Meet established productivity and quality standards while providing consistent, client-centered services. The expected productivity standard for this position is an average of 28 billable hours per week .
