• Assess patients in both inpatient and outpatient environments, crafting individualized treatment plans that cater to their unique needs.
• Administer physical therapy interventions focusing on enhancing mobility, strength, balance, and overall function.
• Collaborate with a multidisciplinary team, including nurses, physicians, case managers, and other rehabilitation professionals, to optimize patient outcomes.
• Educate patients and their families regarding therapeutic goals, discharge plans, and home exercise routines.
• Maintain detailed documentation of evaluations, treatments, and progress in line with facility protocols and guidelines.

Education requirements include a Doctorate in Physical Therapy (DPT) or a comparable qualification from an accredited institution. Licensing requirements include a state license as a Physical Therapist and ideally, a CPR certification. Specialized certifications, although a bonus, are not a strict necessity. We're looking for a team member with previous experience in physical therapy, an ability to navigate complex medical and mobility issues, and outstanding communication and collaboration skills.
