About the Position

Job Title: RN Case Management Profession: RN Specialty: Case Management Duration: 13 weeks Shift: Monday - Friday Hours per Shift: 8:00 AM - 5:00 PM Experience: Minimum 2 years of recent RN Case Management experience License: Texas RN or multi-state RN license required Certifications: BLS certification required Must-Have: - Experience in a Long-Term Acute Care (LTAC) or Specialty Acute Care Client (STACH) setting - Familiarity with MCG and discharge planning across acute care settings - Strong interdisciplinary team (IDT) collaboration skills Description: Coordinates management of care and ensures optimum utilization of resources. Ensures service delivery and compliance with external review agencies. Provides ongoing support and expertise through comprehensive assessment and care planning. Implements plans and evaluates individual patient needs. Enhances the quality of patient management and satisfaction. Promotes continuity of care and cost-effectiveness through case management, utilization review, and discharge planning. Serves as a patient advocate through resource utilization and discharge planning. Addresses the holistic needs of the patient.
