Registered Nurse RN

Overview General Purpose The primary responsibility of your job position is to supervise the day-to-day nursing activities of the facility during your tour of duty. Such supervision must be in accordance with current federal, state, and local standards, guidelines and regulations that govern the facility and may be required by the Director of Nursing (DON) and/or Assistant Director of Nursing (ADON) when applicable, to ensure that the highest degree of quality care is maintained at all times. Responsibilities Essential Duties Note: The statement below identifies essential functions but is not exhaustive. Duties may include related or similar tasks as required. Administrative Functions Assist the Director of Nursing Services in directing the day-to-day functions of the nursing activities in accordance with current rules, regulations, and guidelines for the long-term care facility. Participate in developing, maintaining, and updating written policies and procedures for nursing service operations. Ensure reference material at the nurses’ stations is current; recommend materials to assist the nursing service department in meeting residents’ needs. Keep the Nursing Service Procedures Manual current and reflect daily nursing procedures. Ensure nursing service personnel comply with the Nursing Service Procedures Manual. Provide written and oral reports/recommendations to the Director concerning the operation of the nursing service department. Periodically review policies, procedure manuals, and job descriptions; recommend revisions to the Director. Coordinate nursing services with other resident services to ensure continuity of care. Ensure compliance of nursing service personnel with their job descriptions. Participate in the facility’s quality assurance program for the nursing service department. Support facility surveys conducted by authorized government agencies. Review and update residents’ discharge plans as required; assist in planning the nursing services portion of discharge plans. Meet with nursing staff and support personnel to plan department services, programs, and activities. Admit, transfer, and discharge residents as required. Complete accident/incident reports as necessary. Interpret department policies and procedures to personnel, residents, visitors, and government agencies as required. Perform administrative duties such as completing medical forms, reports, evaluations, studies, and charting as necessary. Protect resident information and report suspected violations of disclosure; report unauthorized attempts to access the facility’s information system. Committee Functions Attend facility committee meetings (e.g., Infection Control, Policy Advisory, Pharmaceutical, Quality Assessment) as required. Prepare written/oral reports of nursing service programs and activities for committees. Provide information to the Quality Assurance and Assessment Committee as requested. Participate in developing the Care Plan Committee agendas. Review discharge plans and make them available to the Care Plan Committee as required. Implement committee recommendations as instructed by the Director. Personnel Functions Ensure adequate staffing of licensed practical and/or registered nurses for each tour of duty to maintain quality care. Ensure adequate certified nursing assistants for daily care needs. Develop work assignments and oversee task execution. Delegate administrative authority to Charge Nurses for assigned duties. Ensure nursing assistants are enrolled in or have graduated from an approved training program; supervise nurse aide trainees. Monitor daily rounds to ensure adherence to nursing standards and report findings to the Director. Monitor absenteeism and address staffing gaps with the Director. Enforce policies and dress codes; review complaints and grievances within the department. Foster good inter-department relations to support resident care. Maintain a positive, warm, and calm environment; lead regular shift meetings to address problems. Review and evaluate staffing and participate in performance evaluations and workforce planning. Administer fair disciplinary actions and manage staffing changes with respect for diversity and inclusion. Report occupational exposures and suspected fraud per facility policies. Ensure computers/workstations are secure when unattended. Nursing Care Functions Inform nursing staff about new admissions and room assignments; ensure rooms are ready for new residents. Greet new admissions and escort them to their rooms; coordinate care with physicians as needed. Plan resident care with residents, families, and physicians; notify when changes in condition occur. Perform daily rounds to observe resident status; ensure direct nursing care is delivered by qualified personnel. Review nurses’ notes for accuracy and alignment with the care plan. Provide direct nursing care as necessary; perform procedures like starting IVs and collecting specimens as ordered. Admit, transfer, and discharge residents as needed; report problems to the Director and assist in corrective actions. Maintain regular reporting to the Director; assist in monitoring seriously ill residents and in discharge planning. Authorize use of restraints only as permitted by policy; ensure proper medication administration procedures and inventory control. Monitor medication passes and treatment schedules; ensure documentation accuracy and adherence to policies. Inspect nursing areas daily for cleanliness and safety; ensure call lights are functioning and residents are checked as needed. Meet with residents/families as necessary and assist with transportation for discharge; inform families of resident deaths as appropriate. Staff Development Develop and schedule in-service training on job performance and nursing education; coordinate with the In-service Director/Educator on annual programs (OSHA, HIPAA, etc.). Plan clinical supervision for nurse aide trainees and continue professional education to maintain licensure. Safety and Sanitation Assist in developing safety standards and ensure exposure control training and PPE provisioning. Enforce hand hygiene, infection control procedures, and fire/disaster drills; participate in infection control programs. Report hazardous conditions and ensure proper labeling and MSDS documentation. Maintain sanitary nursing work areas and patient rooms; ensure safe handling of equipment and supplies. Equipment and Supply Functions Advise on equipment and supply needs; maintain adequate stock of medications and supplies. Work with Maintenance to develop preventive maintenance and safe operation procedures for equipment. Ensure only trained personnel operate nursing equipment and minimize waste; maintain MSDSs for hazardous chemicals. Care Plan and Assessment Functions Contribute to assessments and care plans for residents; involve residents in care plan development. Review plans for goals and revisions; ensure staff awareness of care plans and adherence to them. Assist in planning, scheduling, and revising MDS and RAPs/Triggers; review treatments for alignment with care plans. Budget and Planning Functions Monitor economic conditions and advise on nursing service adjustments; report fraud as needed. Supervisory Requirements Assist with the overall supervision and management of the nursing staff. As an RN, you are delegated the administrative authority, responsibility, and accountability for carrying out your duties. Qualifications Education and/or Experience Must possess, as a minimum, a Nursing Degree from an accredited college or university. Training in rehabilitative and restorative nursing practices. Two years or more experience, preferably in a long-term care facility. Language Skills Must be able to read, write, speak, and understand English. Ability to read technical procedures and policy manuals; effectively present information and respond to questions from managers and employees. Mathematical Skills Ability to apply fractions, percentages, ratios, and proportions to practical situations. Reasoning Ability Must possess the ability to make independent decisions when warranted and solve practical problems; communicate tactfully with residents, families, staff, and regulators; demonstrate leadership and supervisory abilities; plan and implement programs and procedures necessary for quality care. Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of residents; be a team member; maintain knowledge of nursing practices, regulations, and guidelines; demonstrate patience, tact, and enthusiasm; be willing to adopt new methods; relate information about residents’ conditions; not pose a direct threat to others. Certificates, Licenses, Registrations Must possess a current, unencumbered RN license in this state and current CPR certification. Knowledge and Experience Knowledge of PCC preferred. Maintain continuing education/licensing and good standing with the State Board of Nursing. Physical Demands Must be able to move intermittently, speak and write English clearly, cope with mental and emotional stress, see and hear adequately, work independently, and meet health requirements including a medical examination. Must be able to lift at least 25 pounds and assist in evacuations as needed. Work Environment Describes typical work environment and exposure to residents, staff, and conditions; includes potential exposure to infectious materials and chemicals; may require extended hours and on-call duties.
