Key Highlights Weekly pay: $2,198–$2,215 per week Guaranteed hours: 40 per week Contract duration: 13 weeks Shift: 8 hours Start date: 06/15/2026 Work setting: Home health care / patient home visits Travel assignment with dedicated support Responsibilities Administer skilled nursing care for patients of all ages in the home and coordinate with the interdisciplinary team, patient/family, and referring agency Assess home health and hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs Develop, implement, document, and evaluate the Plan of Care to ensure quality and continuity of care Maintain up-to-date patient records, documenting visits, problems, plans, actions, and goals as they occur Educate patients and families on care, medications, safety, and self‑care, using developmentally appropriate teaching methods Supervise and provide clinical direction to home health aides/LPNs to ensure consistent, quality service Assess and reassess pain; apply appropriate pain management techniques and educate on pain control Coordinate referrals to appropriate agencies when needs are identified Maintain infection control practices and ensure compliance with third‑party payer requirements Participate in interdisciplinary team meetings and communicate effectively with patients, families, and health professionals Perform on‑call responsibilities as assigned Requirements Active RN license in Tennessee (current, unencumbered) OASIS experience – Required Homecare Homebase software experience – Required CPR certification – Required 1–2 years RN clinical care experience; 2+ years Home Health experience – Required Education: ADN or Nursing Diploma program graduate; BSN preferred CHPN certification – Preferred Current driver’s license and reliable transportation with adequate auto insurance Benefits and Support Guaranteed hours and weekly pay Housing assistance and travel support 24/7 recruiter and travel support Dedicated assignment support and onboarding assistance Fast credentialing and extension opportunities Nationwide assignments and ongoing professional development
