Job Overview Address: 2450 N Orange Blossom Trail, Kissimmee, FL 34744|Schedule: Full time, Day shift (8:00‑5:00, Monday‑Friday) with weekend on‑call rotation. Location: Kissimmee or St.Cloud. Responsibilities Admit new patients, conduct comprehensive initial evaluations, and perform home safety assessments. Establish individualized care plans, set goals, and continuously evaluate patient response. Modify care plans as needed, report observations to the attending physician, and adjust treatment accordingly. Educate patients and families on the physical therapy program, including use of prosthetic, orthotic, and assistive devices. Participate in interdisciplinary team meetings, collaborate on patient care plans, and attend team conferences. Supervise and plan home health aide activities in the absence of other healthcare professionals. Maintain clinical records according to established standards and within required time frames. Participate in performance improvement programs, orientation, in‑service, and staff meetings as scheduled. Display flexibility in accepting assignment changes, weekend coverage, and call rotation. Competency in required documentation and patient care procedures as a PT. Effective oral and written communication in English. Ability to make safe, independent judgments and decisions in emergencies. Knowledge of identification, prevention, remediation, and rehabilitation of patients with impairments, functional limitations, or disabilities. Flexibility and positive interaction with associates. Education & Licenses Doctorate in Physical Therapy (Preferred). Physical Therapist (PT) license—Required. Basic Life Support – CPR (BLS) certification—Required.
