Per Diem Direct Support Professional - Levittown, NY

Position Overview Per Diem Direct Support Professionals needed at our Levittown location. This role offers flexible scheduling: you can pick shifts that fit your availability 24/7. Minimum of 16 hours per month after completing required paid agency training. Key Responsibilities Assist individuals in maximizing their strengths, interests, and abilities; uphold personal rights, exhibit patience and tolerance while providing emotional support and building positive relationships. Support people to meet personal goals that characterize quality of life; engage in respectful communications and conversation using person‑first language; encourage socializing and facilitate activities that provide community connections. Supervise, protect, and care for individuals at all times, including maintaining program guidelines, facilitating fire safety training, and escalating signs of self or other harm per an individual’s behavior support plan. Provide appropriate supervision and crisis management while adhering to applicable policies, procedures and regulations. Deliver support in a variety of settings, following each person’s individualized service and behavior support plan, utilizing support skills for people with varying levels of need, and demonstrating OPWDD Core Competencies. Implement person‑centered, individualized plans, following levels of supervision and safeguards—including protective oversight (IPOPs) and behavior support plans—as well as staff action and/or life plans for all people we support. Maintain a clean, safe and organized therapeutic environment; implement safe and appropriate hand‑ing procedures when working with individuals and follow infection control protocols. Assist with a variety of tasks for people we support, including personal care and activities of daily living (ADLs); intimate care and personal hygiene; laundry and housekeeping; medication administration; teaching/developing skills; advocacy; communication; conflict resolution; ethical decision making. Prepare balanced meals and snacks in accordance with dietary restrictions; transport individuals to recreational activities, medical appointments, home visits, volunteer and/or employment sites. Assist people with making purchases and safeguard personal allowance funds and other funds provided. Develop an understanding of specialized clinical knowledge for person‑specific needs, including adults, challenging behaviors, autism spectrum disorders, and complex medical needs. Implement behavior support plans as written by behaviour intervention specialist; collect, record and summarize data on observable behavior; observe and report changes or other signs and symptoms to clinical or leadership teams. Serve as a member of a highly effective team; provide feedback and communicate programmatic needs; maintain a solution‑oriented approach to conflicts. Complete and maintain all necessary trainings and certifications, including specialized training based on program and position to ensure compliance with YAI policies and all applicable rules or regulations. Review communication logs daily and record events, progress notes, services, and other information; manage documentation via digital health records (e.g. Digital Agency, eMAR). Comply with all federal, state, local and other relevant regulatory agency requirements, including HIPAA. Report maintenance, safety concerns and facility issues to program supervisor or appropriate designee. Consult with program leadership, clinical and direct‑support staff to ensure high‑quality care and attend team meetings. Utilize YAI and OPWDD training techniques—including verbal, environmental, and physical interventions—to respond timely and appropriately to challenging or self‑injurious behaviors. Provide coverage for other direct‑support staff as requested, which may include assisting at another YAI or off‑site location; remain on shift to maintain minimum coverage levels for program. Participate in development of treatment plans when required. For certain assignments, operate YAI motor vehicle to transport individuals to and/or from programs, residences, community or other locations on a regular basis with appropriate credentials. Perform all other duties as assigned. Qualifications Must be at least 18 years of age. Ability to meet essential physical demands of the position: frequent walking, sitting, standing, bending, twisting, stooping, kneeling, crouching, pushing, pulling, reaching; handle objects; assist with lifting/moving individuals with mechanical lifts or other equipment; lift or move up to 25 pounds on occasion. Ability to implement emergency interventions as per the person’s behavior support plan. Effective written and oral communication skills with the ability to document services for billing and tracking. Enthusiasm for working with I/DD and willingness to take initiative; demonstrate positivity, patience, empathy and compassion. Ability to assist with a variety of ADLs and exposure to bodily fluids. Basic computer skills, including email and utilization of Workday and related systems (DA, etc.). Hold or obtain and maintain Strategies for Crisis Intervention and Prevention – Revised (SCIP‑R) within one year of appointment. Hold or obtain and maintain additional certifications as required by program assignment (e.g., Choking – Return Demonstration, Approved Medication Administration Personnel, Diabetes Care, CPR, Tube Feeding) within one year of appointment. Ability to develop and maintain professional relationships aligned with YAI’s mission. Remain calm in stressful situations and work in a high‑energy, fast‑paced environment. Work beyond regularly scheduled hours, shifts, and holidays or during inclement weather to maintain minimum coverage requirements, unless authorized otherwise. For assignments requiring transportation of persons by vehicle, hold or obtain and maintain a valid driver’s license and van certification. Preferred Qualifications High school diploma or equivalent. Work or life experience in the I/DD field is a plus (not mandatory for DSP1 positions). Three or more years of direct‑support professional experience with I/DD population for elevated DSP positions; obtain required YAI certifications within six months of appointment. Compensation & Benefits $18.00/hour – $19.05/hour, depending on qualifications and experience.
