Essential job duties include:

Perform comprehensive in-person assessments, leading and partnering with the interdisciplinary team (IDT) to develop a personalized care plan for each participant
\n Coordinate discharge for acute, short-term, and long-term placement
\n No hospital rounds focus on comprehensive care, seeing an average of 8 patients a day

Job requirements include:

\n
• Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical
\n
• Education:
\n
• Master's or doctorate Degree
\n
• Board Certification preferred
\n
• Active state physician license without restrictions
\n
• State CDS certification and DEA registration
\n
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred
\n
• We are seeking Primary Care Physicians in Internal Medicine, Family Medicine, Emergency Medicine or Geriatrics
\n
• Must be eligible for enrollment to provide services to Medicare and Medi-Cal participants. Board certification preferred.
