Responsibilities
- Provide direct patient care to residents in a long-term care setting, under RN supervision.
- Administer prescribed medications and document administration accurately.
- Monitor resident status, recognize changes, and communicate updates to the care team.
- Assist residents with activities of daily living and mobility while ensuring safety and comfort.
- Document care activities thoroughly in the electronic health record and coordinate with the interdisciplinary team.
- Respond to call lights and urgent situations following established protocols.

Requirements
- Active LPN license or eligibility to practice in New Mexico; applicable to the assignment.
- Ability to work a 3x12-hour shift schedule in a long-term care environment.
- Demonstrated ability to perform essential nursing duties in a skilled nursing setting and to document care accurately.
- Strong communication skills and ability to collaborate with an interdisciplinary team.
