Certified Registered Nurse Practitioner (CRNP) – HospicePrimarily In?Office | Regional Team | No Nights or WeekendsSerenity Hospice is seeking a compassionate and experienced Certified Registered Nurse Practitioner (CRNP) to join our dedicated regional hospice team. This is a primarily in?office position focused on meaningful, face?to?face clinical visits with hospice patients while collaborating closely with physicians and interdisciplinary care teams.In this role, you will play a vital part in ensuring exceptional end?of?life care by conducting required hospice face?to?face evaluations and supporting high?quality, compliant, and compassionate patient care across our service region.Position HighlightsMostly in?office roleFace?to?face patient evaluations are the primary focusRegional team support across care centersNo nights, weekends, or on?call requirementsConsistent schedule that supports work–life balanceKey ResponsibilitiesConduct face?to?face hospice patient visits to evaluate continued eligibility in accordance with CMS Hospice Eligibility GuidelinesCollaborate with the Hospice Medical Director and attending physicians to review patient status and adjust care plans as appropriateProvide clinical leadership and guidance to the interdisciplinary hospice teamSupport symptom management strategies and clinical decision?makingAuthorize medications and treatments in accordance with state regulations and patient care needsEnsure accurate, timely, and compliant clinical documentationParticipate in Interdisciplinary Group (IDG) meetings and care planning discussionsWork primarily from an assigned Serenity Hospice office, with occasional local travel to patient homes or care facilitiesQualificationsCurrent and active CRNP license in PennsylvaniaHospice or palliative care experience requiredThorough understanding of CMS Hospice Eligibility GuidelinesStrong interpersonal, organizational, and documentation skillsAbility to work collaboratively within an interdisciplinary teamWillingness to travel locally within the region, as neededWhat We OfferNo nights or weekendsFlexible scheduling (Full?Time or Part?Time; minimum 25 hours/week)Competitive hourly rate: $60–$65 per hourComprehensive benefits package, including:401(k)Health, dental, and vision insurancePaid time offSupportive leadership and collaborative team environmentOpportunities for professional development and continuing educationThe opportunity to make a meaningful difference in patients' and families' livesLocations & Regional CoverageSerenity Hospice operates four Care Centers in Pennsylvania.
