Home Health Registered Nurse Case Manager

CenterWell Home Health, part of Humana Inc., is seeking a Home Health RN Case Manager to deliver compassionate, patient-centered care in the home. In this role, you’ll manage a caseload of seniors and adults with chronic conditions, performing comprehensive assessments, skilled nursing interventions, and medication management. You’ll develop and update individualized care plans, coordinate with an interdisciplinary team, and educate patients and caregivers to support safety and independence.
