• Provide acute case management services
• Collaborate with CM, SW, and SNF coordinator teams
• Utilize Meditech and Midas EMR systems
• Coordinate care plans and resource management for patients

Requirements of the RN - Case Management - Travel

• Valid RN license
• Acute case management experience
• Familiarity with Meditech and Midas systems
• Previous experience with HCA facilities preferred
