This position also works with the Registered Nurse to ensure the operational running of the medical clinical area and patient communication within the electronic health record system.Main Responsibilities :Clinical Floor OperationsPrepare clinical area for building opening or closing in accordance with Finger Lakes Community Health hours of operationRedirect nursing team tasks to ensure an efficient patient flowClean and sterilize instruments, equipment, and clinical areas according to clinical infection control proceduresRestock rooms with medical suppliesAssess inventory and place order for medical suppliesPerform Quality Control measures including but not limited to: Monthly control testingEquipment inspectionsAttend morning huddle meeting with patient care team to address patient concernsFollow-up on referral progress for patients initiated by providerReview patient information during pre-visit planning to ensure appointments are appropriately scheduled and needed information has been receivedClinical ServicesObtain patient vitals and history during patient rooming processNotify nurse or provider of abnormal findings during patient rooming processAssist with minor procedures and exams as outlined in NYS clinical scope of practicePerform patient screenings as outlined in NYS clinical scope of practiceComplete phlebotomy and lab collection and handling as outlined in NYS clinical scope of practiceConfirm patient charts have written orders for medications and/or injections from the providerAdminister injections and immunizations as directed by ordering providerComplete medication reconciliationReview and send medication refill requests to patient's providerInform patients or designated persons of physician orders, changes in care plan, test results, etc. as outlined in NYS clinical scope of practiceMedical Record MaintenanceScan clinical documents from patient or clinical staff into patient chartChart services in patient record including but not limited to: Referrals, medications, vaccinations, injections, screenings, lab orders/results, patient history, vitalsChart and lock notes within timely manner in accordance with Finger Lakes Community Health policies and proceduresAddress telephone encounters, actions, and referrals in the electronic health record system in accordance with Finger Lakes Community Health policies and proceduresEducation and Qualifications:High School Diploma or equivalentA current unrestricted NYS LPN license and CPR certification requiredPrior experience a plus, but not requiredThe ability to work flexible hours -- some evenings are requiredThe ability to communicate fluently in Spanish and English preferred, but not requiredA valid New York State driver's license in compliance with FLCH drivers' policyPhysical Requirements:Must be able to hear and communicate with clients and staff via phone, web media, and in person.Must be able to lift up to 40 lbs.Must be able to sit for extended periods of timeMust be able to kneel, crawl, or bend into awkward spacesOSHA Level 1
