RN Case Manager

The RN Case Manager is responsible for managing an acute care caseload of medical patients, providing skilled nursing care, comprehensive patient assessments, medication administration, treatment planning, and coordination with the interdisciplinary team. This role requires strong communication and collaboration skills to ensure effective case management and optimal patient outcomes. A minimum of two years of relevant experience is required. Responsibilities:

• Manage acute care medical patient caseload effectively.
• Perform thorough patient assessments and develop individualized treatment plans.
• Administer medications and monitor patient responses.
• Coordinate care and communicate with interdisciplinary team members.
• Provide case management including discharge planning and patient education.
• Document patient care activities accurately and timely.
• Collaborate with patients, families, and healthcare providers to optimize care delivery.

• Active Registered Nurse license in the state of employment.
• Minimum of two years experience in hospital, home health, or hospice care.
• Strong clinical assessment, medication administration, and treatment planning skills.
• Excellent communication and interpersonal skills.
• Bachelor's degree in Nursing or related field.
• Driver's license, car insurance, and reliable automobile for field work.

• Experience in acute care settings or case management.
• Familiarity with interdisciplinary team collaboration in healthcare environments.

• Ability to work in fast-paced acute care environments.
• May require occasional field visits requiring personal transportation.

• Work schedule and shifts to be determined based on patient caseload and facility needs.
