Job Responsibilities Collaborate with physicians and preceptors to implement the assessment plan, evaluate outcomes, and follow up on care plans for patients. Initiate ongoing systematic assessments of patients' physical, emotional, social, educational, and functional needs. Interpret assessment data to develop detailed nursing care plans including problem identification, nursing diagnoses, orders, interventions, goals, and discharge planning. Participate as a productive member of the interdisciplinary team in assessment, planning, implementation, and evaluation of rehabilitation goals. Ensure medical records are complete, factual, accurate, and timely. Minimum Qualifications Graduate of an accredited nursing program. Valid State RN license required at hire.
