Nashville, TN | Hybrid Schedule (work + local travel to facilities) Direct Hire | $65,000–$70,000 + Benefits GSD Search is partnering with a growing healthcare and community services organization to identify a Case Manager who is passionate about helping individuals navigate complex care needs and access critical support services. This position offers a flexible hybrid schedule, typically consisting of approximately three days in the office and two days spent visiting local facilities and community partners throughout the Nashville area. Travel days generally allow for remote administrative work and scheduling flexibility.

Position Overview

The Case Manager will serve as a key advocate and resource for clients, coordinating services, monitoring progress, and collaborating with caregivers, providers, and community partners to ensure individuals receive the support necessary to achieve positive outcomes. This role is ideal for someone who enjoys balancing relationship-building, care coordination, field-based engagement, and documentation responsibilities.

Responsibilities

• Manage and coordinate services for an assigned caseload of clients.
• Conduct assessments and assist with the development of individualized care and service plans.
• Serve as a liaison between clients, families, providers, facilities, and community resources.
• Monitor client progress and ensure services remain aligned with established goals.
• Complete required case notes, documentation, and reports in a timely and accurate manner.
• Conduct routine visits to local facilities and community locations throughout the Middle Tennessee area.
• Identify barriers to care and advocate for appropriate services and resources.
• Participate in interdisciplinary meetings and collaborate with internal and external stakeholders.
• Maintain compliance with organizational policies and applicable regulations.

Qualifications

Required

• Master of Social Work (MSW) degree required
• 2+ years of experience in case management, care coordination, social work, behavioral health, healthcare, community services, or a related field.
• Strong communication, organization, and documentation skills.
• Ability to build trust and effectively engage with diverse populations.
• Valid driver's license and reliable transportation for local travel.

Preferred

• Experience supporting individuals with complex medical, behavioral health, developmental, or social service needs.
• Familiarity with community-based services, care planning, and resource coordination.
• Experience working with healthcare providers, residential facilities, or state-funded programs.
