Palms Medical Group (PMG) is seeking a Licensed Clinical Social Worker to deliver behavioral health services to referred PMG patients, including crisis intervention, brief psychotherapy, and health management. You will collaborate with the care team to coordinate treatments and referrals. Responsibilities include maintaining records, after-hours availability, and participation in QA/QI and community health programs, in a mission-driven, non-profit FQHC environment across North Florida. #J-18808-Ljbffr Palms Medical Group
