Job Details Main Office location: Cranston, RI Service Area: Cranston, Warwick, West Warwick, Coventry, East Greenwich, Providence. Job Type: Full Time Schedule: Monday to Friday Responsibilities Assess and evaluate patient’s status. Write and initiate plan of care. Regularly re‑evaluate patient and family/caregiver needs. Participate in revising the plan of care as necessary. Develop a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions, including the patient and the family in the planning process. Initiate appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the physician in the physician’s plan of care. Submit accurate and relevant clinical notes regarding patient’s condition and care given. Record pain/symptom management changes/outcomes as appropriate. Communicate with the physician regarding the patient’s needs and report changes in the patient’s condition; obtain/receive physicians’ orders as required. Provide and maintain a safe environment for the patient. Attend interdisciplinary group meetings. Additional Duties Participate in on‑call duties. Ensure that arrangements for equipment and other necessary items and services are available. Supervise ancillary personnel and delegate responsibilities when required. Assume responsibility for personal growth and development and maintain and upgrade professional knowledge and practice skills through attendance and participation in continuing education and in‑service classes. Fulfill the obligation of requested and/or accepted case assignments. Actively participate in quality assessment performance improvement teams and activities. Qualifications 2+ years’ nursing experience, preferably in geriatrics. Degree in Nursing from an accredited institution. Proficient with electronic medical records. Excellent bedside manner and communication skills. Working knowledge of health‑care laws and regulations. Maintain current BLS CPR certification. Possess a valid driver’s license and own a reliable, insured vehicle.
