Licensed Practical Nurse

Job Details Location: White Oaks Assisted Living and Memory Care, Lawton, MI 49065 Summary: Responsible for enhancing and improving the SRIM experience in heartful care for residents and their families. Demonstrates and promotes SRIM’s core value of the golden rule—treating residents, families, visitors and associates the way we would like to be treated. Performs assigned nursing procedures for the comfort and well‑being of residents, such as assisting with admissions, activities of daily living when needed, administering specified medications, supervising non‑licensed nursing staff, and participating in staff evaluations. Duties and Responsibilities Demonstrates excellent decision‑making skills and understands the impact of those decisions. Sets and manages priorities. Accepts calls from staff during nights and weekends. Meets with on‑coming and departing licensed practical nurses to discuss residents’ conditions and shift occurrences. Ensures communications are clear, accurate and timely. Counts controlled substances with outgoing nurse and assumes responsibility for accurate counts before accepting keys. Checks, orders and administers prescribed medication with 100% accuracy. Ensures correct dosage by never pre‑pouring medications. Demonstrates knowledge of important medication interactions and side effects. Ensures medication cart is locked at all times. Obtains pertinent information and completes all required forms upon resident admission following policy and procedure. Maintains the ECC and LNS logs, service plan, progress notes, monthly summaries and follows healthcare provider orders. Assists in completing evaluations for residents when needed. Provides hands‑on care as needed to meet residents’ needs. Works with the ADORS to review and investigate incident reports and recommends appropriate action and follow‑up to the DORS/executive director. Understand and adhere to ALF regulations regarding assisted living healthcare laws. Maintains a safe and secure environment free of hazards that could cause harm or falls for residents, families, visitors and associates. Always practices good body mechanics and safe work habits. Follows established policy and procedures in emergency situations. Provides daily feedback to nursing associates on performance. Participates in associate performance evaluations when requested by the DORS. Ensures associates participate in activities for residents as described in the activity calendar. Responds promptly and courteously to residents’ requests for assistance, including phone calls and visitor requests. Maintains a clean, orderly environment for all residents. Supports a dignified and caring atmosphere for residents, families, visitors and associates. Wears clean and appropriate dress per established policy. Complies with HIPAA regulations. Recognizes essential personnel status, especially during emergencies. Takes initiative, seeks additional information, and offers solutions or suggestions. Performs all other reasonable duties as assigned or requested. Willingly takes on special projects or assignments outside of regular duties. Supervisory Responsibilities Directly supervises employees in the Resident Services/Nursing Department. Responsibilities include training, planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints; and resolving problems, in accordance with the organization’s policies and applicable laws. Physical Demands Regularly required to stand, walk, use hands, reach with hands and arms, talk or hear; frequently required to stoop, kneel, crouch or crawl; occasionally required to sit, climb or balance, and taste or smell; must occasionally lift and/or move up to 50 pounds. Qualifications High school diploma or GED; 1–2 years related experience or training, or equivalent combination of education and experience. Valid state‑specific nursing license. Language ability: read, analyze and interpret business periodicals, professional journals, technical procedures or regulations; write reports, correspondence, and procedure manuals; present information and answer questions from managers, clients, customers, and the general public. Mathematics: add, subtract, multiply, divide, compute rate and ratio, and interpret bar graphs. Reasoning: solve practical problems and handle varied situations with limited standardization; interpret instructions in written, oral, diagrammatic, or schedule form.
