Job Title: Hospice RN Case Manager

Location: Grand Junction, CO, United States
Job Type: Contract
Shift: Day Shift | Monday–Friday | 8:00 AM – 5:00 PM
Duration: 12 Weeks

Job Overview

"Navitas Healthcare, LLC" is seeking an experienced and compassionate Registered Nurse Case Manager to provide high-quality hospice and end-of-life care. This role focuses on patient-centered care, symptom management, family education, and coordination of hospice services across multiple care settings.

Key Responsibilities

Manage and coordinate individualized hospice care plans
Perform patient assessments and symptom management evaluations
Educate patients and caregivers regarding treatment goals and disease progression
Coordinate interdisciplinary hospice care services
Provide nursing care in homes, assisted living facilities, nursing homes, and group homes
Support patients and families throughout all stages of hospice care
Maintain accurate documentation and patient records
Collaborate with physicians, social workers, and hospice care teams
Ensure compliance with hospice care standards and safety protocols

Required Qualifications

Active Registered Nurse (RN) License required
Minimum 2 years of nursing experience required
Hospice or case management experience preferred
Strong communication, assessment, and critical thinking skills
Ability to work independently and within interdisciplinary teams
