Position Summary Assesses, plans and provides physical therapy services to clients in their homes in accordance with Medicare, Arizona, and Federal regulations and guidelines. Contributes and participates in care coordination and patient-centered care as a part of an interdisciplinary clinical care team of nurses, therapists, CNAs, and MSWs. Minimum of one year's experience with a preference for prior Home Health experience. CPR certification. Fingerprint clearance card. TB test result. Licensed driver with automobile insured in accordance with state and/or Agency requirements with a good driving record. Demonstrates strong verbal and written communication skills.
