Licensed Practical Nurse - Hospice (Full-Time, M-F)

Location: Delaware, OH; Marion, OH; and Surrounding Counties Employment Type: Full-Time Shift: 8 AM – 5 PM (flexible) Schedule: Monday – Friday Pay Rate: $31-34 (negotiable based on experience) Providing care and patient follow-up in coordination with the RN/Case Manager or On-Call RN/Case Manager. The Hospice LPN provides hospice nursing services while offering emotional, social, and spiritual care and support to patients and their caregivers. Scope of Work Nursing Assist the RN/Case Manager in providing hospice care in accordance with each patient’s individualized plan of care. Assist the RN/Case Manager with specialized procedures as directed by the attending physician and Hospice Medical Director. Administer medications, perform medication reconciliation, and provide nursing procedures or special treatments within the scope of LPN practice in the State of Ohio. Documentation Submit complete, accurate, and relevant clinical documentation in a timely manner. Assist the RN/Case Manager with the preparation, implementation, and ongoing evaluation of the patient’s plan of care. Promptly communicate changes in the patient’s condition or needs to the RN/Case Manager. Patient & Family Support Assist in providing and reinforcing education to patients and family members regarding the dying process and hospice care. Obtain relevant medical history from the patient and/or family members, particularly as it relates to the patient’s current condition. Provide compassionate support to patients and caregivers throughout the hospice journey. Communication Maintain clear and professional communication with patients, families, caregivers, and members of the interdisciplinary team. Communicate changes in patient condition, concerns, and care needs to the appropriate RN/Case Manager or On-Call RN/Case Manager. Technology Utilize the Electronic Health Record (EHR), email, computers, and mobile devices as required for patient care, documentation, and communication. Qualifications Graduate of an accredited School of Nursing. Current and unencumbered Licensed Practical Nurse (LPN) license in the State of Ohio. Ability to administer medications, perform medication reconciliation, and complete nursing procedures and special treatments within the applicable scope of nursing practice. Minimum of one year of LPN experience; hospice experience preferred. Strong assessment and problem-solving skills. Strong organizational and time-management skills. Knowledge of symptom management and hospice care principles preferred. Excellent verbal and written communication skills. Valid driver’s license with access to a reliable and insured automobile. Education & Licensure Graduate of an accredited School of Nursing. Current, unencumbered State of Ohio Licensed Practical Nurse (LPN) license. Job Conditions & Physical Requirements Position requires regular walking, sitting, standing, stooping, and other physical activities associated with providing patient care. Must be able to lift up to 50 pounds. Travel is required and may account for up to 100% of the position’s work-related activity. Must be able to travel throughout the assigned service area.
