Per Diem Physical Therapist Assistant – Skilled Nursing Facility

Location: Dearborn

Shift: Per Diem

We are currently seeking a dedicated Per Diem Physical Therapist Assistant (PTA) to join our Skilled Nursing Facility team. This flexible, as-needed position is ideal for an individual who is passionate about helping residents achieve their highest level of function and independence.

Key Responsibilities

• Provide direct patient treatment under the supervision of a licensed Physical Therapist, following established plans of care

• Assist in the implementation and modification of treatment plans to promote optimal functional outcomes

• Monitor and document patient progress, reporting changes in condition to the supervising Physical Therapist

• Instruct patients, families, and caregivers in exercise programs and safe mobility techniques

• Maintain accurate, timely, and compliant documentation of care and treatment provided

• Adhere to all facility policies, including quality improvement, safety, and infection control standards

• Communicate effectively with the interdisciplinary care team, ensuring continuity and quality of care

• Support a positive, team-oriented work environment and contribute to patient satisfaction

• Participate in ongoing professional development and in-service training as required

• Perform other related duties as assigned

Education & Certification Requirements

• Graduate of an accredited Physical Therapist Assistant program

• Current state license as a Physical Therapist Assistant in good standing

Qualifications

• Knowledge and skills necessary to provide care within a Skilled Nursing Facility (SNF) environment

• Previous SNF experience preferred

• Understanding of applicable state, federal, and professional standards

• Strong communication, organization, and time management abilities

• Compassionate, patient-centered approach with a strong sense of empathy

• Excellent teamwork and interpersonal skills

• Positive and professional demeanor

• Ability to lift at least 40 lbs.

• Frequent walking, standing, pushing, pulling, reaching, crouching, stooping, bending, and stretching

• Ability to assist with patient transfers and perform necessary documentation tasks

• 80-bed skilled nursing facility (smaller, team-oriented environment)

• Unlike facilities that outsource therapy services, our rehabilitation team is fully in-house, promoting consistency and communication.

• Lower-acuity care setting (non-vent, non-trach facility)

• Continuity of care across Independent Living, Assisted Living, and Skilled Nursing on one campus

• Staff build long-term relationships with residents across levels of care

• Each resident has a   private room (no shared rooms), supporting dignity and comfort

• Convenient central location with easy access via multiple major highways
