As a Wellness Nurse LPN, you play a central role in monitoring resident health, supporting medication oversight, and maintaining clinical quality across the community.What You'll Do:Conduct move-in, 3-day, semi-annual, and change-of-condition assessmentsPerform monthly wellness visits and monitor health trendsFollow up on falls, incidents, and hospital returnsTranscribe physician orders and ensure accurate MAR/TAR documentationComplete medication audits and oversee medication compliancePartner with families, physicians, and the Resident Care DirectorSupport infection control and regulatory standardsEducate and guide care staff on documentation and medication practicesYou help prevent decline through proactive assessment, early intervention, and strong clinical oversight.What We're Looking For:Active LPN license in good standingGraduate of approved nursing program1+ year experience in senior care or clinical settingStrong assessment, documentation, and communication skillsLicensed Practical Nurse (LPN/LVN) – Support Resident Health and WellbeingLicensed Practical Nurse LPN LVN | Nursing | Assisted Living | Senior LivingPay Range: 29.15 - 38.90Ready to take the next step and make a bigger impact than you ever imagined?
