This is a 13-week contract position.

Schedule

Day shift, 5x8 hours (08:00 - 16:30), Monday through Friday. No weekends or holidays required.

Job Description

The Case Manager RN will be responsible for coordinating care for all medical and surgical patients within various units, including ICU and IMC. This role involves ensuring comprehensive patient management and facilitating the delivery of necessary healthcare services.

Responsibilities

• Manage patient care and coordinate with healthcare providers.
• Conduct admission assessments and establish plans of care.
• Perform discharge planning and ensure continuity of care.
• Assess medical necessity and manage appeals and denials.
• Conduct ongoing reviews for continued stay and compliance with CMS guidelines.

Required Experience / Certifications / Licensure

• Active RN license in Maryland or compact state.
• Basic Life Support (BLS) certification required.
• Experience with Epic charting system is required.
• Healthcare experience in community hospitals is preferred.
• Care Management certification is preferred.
