Responsibilities
- Perform initial evaluations and ongoing assessments of residents to determine physical therapy needs.
- Develop, implement, and monitor individualized treatment plans aimed at improving mobility, strength, balance, gait, and functional independence.
- Provide direct patient care through therapeutic exercises, modalities, and restorative interventions, while ensuring safety and infection control protocols.
- Document patient progress, update treatment plans, and liaise with the facility’s interdisciplinary team to support discharge planning and care transitions.
- Educate residents and families on home programs, fall prevention, posture, and activity modification; ensure clear and accurate clinical notes.

Requirements
- Active Physical Therapist license in Washington state
- Ability to work effectively in a skilled nursing facility environment
- Prior experience in geriatric rehabilitation or SNF settings preferred
- Strong documentation, communication, and clinical reasoning skills
- Availability to start on the specified date for a 13-week travel assignment
