We get young children off to a good start, launch youth into adulthood, stabilize and strengthen families and unlock potential for children and adults with special needs.SCO Family of Services welcomes you to join our family and help us work together to carry out our mission to build a stronger tomorrow.PURPOSE OF THE POSITION: The Care Managers service those who are enrolled in Medicaid, have Two or more Chorionic Conditions, HIV/AIDS, Sickle Cell, Serious Emotional Disturbance (SED), Serious Mental Illness (SMI) or Complex Trauma; ages 0-21 (Childrens Program) & 18 to end of life (Adult Program). The Care Manager plays a critical role in achieving the overall goals for coordination, integration and partnership in the delivery of care to children, adults and families with complex needs.REPORTS TO: Care Manager SupervisorSUPERVISES: Not ApplicableSPECIFIC RESPONSIBILITIESThe Health Home Care Manager delivers all six NYS Department of Health (NYS DOH) Health Home Core Services, including:Comprehensive Care ManagementCare Coordination and Health PromotionComprehensive Transitional CareIndividual and Family SupportReferral to Community and Social Support ServicesUse of Health Information Technology to Link ServicesAdditional responsibilities include:Conducting NYSDOH-required assessments and developing compliant, person-centered Plans of CareCoordinating medical, behavioral health, substance use, and social service supportsEngaging members through outreach, advocacy, and ongoing follow-upCollaborating with hospitals, providers, Manage Care Organizations (MCO), and community partnersMaintaining accurate, timely documentation in NYSDOH-approved systemsMeeting Health Home productivity, engagement, and quality benchmarksParticipating in case conferences, audits, supervision, and trainingRELATIONSHIP WITH OTHERS & IN THE WORKPLACEDemonstrates professionalism, maintains appropriate boundaries, and follows the agency's Code of ConductPromotes a positive, respectful, and productive work environmentUpholds the agency's mission and vision in all interactions with staff and families servedIn addition, employees need to possess the following characteristics:Strong team player with effective communication and leadership skillsMaintains confidentiality, professional boundaries, and appropriate guidance of staffBuilds trusting, supportive, and culturally responsive relationshipsBelieves in growth and collaboration with individuals and families servedRemains calm and effective during crises and intervenes appropriatelyWORKING CONDITIONSThis is a hybrid office- and field-based position requiring regular travel throughout Suffolk and Nassau Counties and select boroughs of New York City. Travel may occur multiple times per week based on program needs.The role includes scheduled in-office days, with meetings and trainings held in person at our Brooklyn or Dix Hills locations, as well as virtually as needed. While every effort is made to assign care managers to cases within their designated catchment area whenever possible, flexibility to travel for service delivery is required.
