Laurel Creek Health & Rehab is seeking a dedicated, experienced, and detail-oriented RN MDS Coordinator to join our team. In this key leadership role, you will work closely with residents, providers, and the interdisciplinary care team to ensure accurate assessments, regulatory compliance, and exceptional care planning.

Qualifications:

• Currently licensed in KY as RN.

• Bachelor’s degree in nursing preferred.

• One year of MDS experience preferred.
