Experienced RN Case ManagerThe shift for this position is from 8:00 AM to 5:00 PM, Monday through Friday, with a start time ranging from 8:00 AM to 9:00 AM. The total work hours will be approximately 8.5 hours per day.Job DescriptionWe are seeking an experienced RN Case Manager to provide high-quality patient care through effective case management practices. The ideal candidate will possess a solid background in acute care and demonstrate the ability to coordinate and manage patient care comprehensively.ResponsibilitiesConduct patient assessments and develop individualized care plans.Collaborate with the healthcare team to ensure care delivery aligns with best practices.Monitor patient progress and adjust care plans as necessary.Support patients and their families through education and community resource navigation.Required Experience / Certifications / Licensure2+ years of acute care Case Manager experience required.Active PA RN license required.Bachelor of Science in Nursing (BSN) is required.Assignment DurationThis is a contract position with guaranteed hours of 40 hours per week for a total of 88 weeks.Why ARMStaffing?At ARMStaffing, we take care of our employees!
