RN - Hospice

Hospice Nurse - Home Health

This is a 13-week contract position.

Full-time schedule: 5 days a week, 8 hours per shift, with mainly daytime hours. May include after hours and/or weekends based on agreement.

The Hospice Nurse will provide compassionate care to patients in their homes, ensuring comfort and support during their end-of-life journey. This role requires recent experience in hospice care and the ability to navigate HomeCare HomeBase systems effectively.

Responsibilities:

• Conduct patient assessments and develop care plans.
• Administer medications and manage symptoms effectively.
• Provide education and support to patients and families regarding end-of-life care.
• Collaborate with healthcare team members to ensure high-quality care.
• Document care provided in HomeCare HomeBase or as trained.

Required Experience / Certifications / Licensure:

• Recent experience in hospice care is required.
• HomeCare HomeBase experience preferred; training may be provided for the first two weeks.
• Must have active RN license and appropriate certifications for the role.

Position covers a 60-mile radius from the facility. May be required to assist in other territories as needed.
