Duration: 14 Weeks
Shift & Schedule: Day Shift (5x8 Hours) | 09:00 AM – 05:30 PM | 40 Hours Per Week
Pay Rate: $90 - $95/hr

Position Overview:

Navitas Healthcare, LLC is seeking a Nurse Supervisor for an exciting Contract opportunity in New York, NY.

Responsibilities

• Assist with planning, organizing, directing, coordinating, and evaluating nursing services within the Transitional Care division.

• Develop and implement programs to improve patient care quality and employee performance.

• Monitor patient care performance and collaborate with Nurse Managers to implement corrective actions.

• Perform Director of Nursing responsibilities in their absence.

• Participate in developing policies, standards, and educational initiatives for Care Management services.

• Provide leadership and oversight to Accountable Care Managers (RNs), Care Coordinators (CLWs), clerical, and ancillary staff.

• Coordinate transitional care initiatives to reduce 30-day hospital readmissions and improve patient outcomes.

• Support high-risk patient populations transitioning from inpatient Med/Surg units and the Adult Emergency Department to community-based care.

• Develop and maintain systems for tracking medication errors and counseling activities.

• Ensure care coordination supports safe and effective patient transitions into community settings.

Required Qualifications

• Minimum 7 years of nursing experience required.

• Minimum 5 years of supervisory experience required.

• Minimum 2 years of experience in discharge planning and utilization review required.

• Experience providing Associate Director-level supervision to a Care Management Department required.

• Bachelor’s Degree in Nursing from an accredited Nursing Program required.

• Active RN License required.

• CCM Certification required.

• PRI Certification required.

• Primary Source Verification of License and Registration of Registered Professional Nurse from NYS Department of Education (NYSED) required.

• BLS Certification required.

• Strong care management, transitional care, leadership, communication, and organizational skills.

• English proficiency (Speak, Read, Write) required.

Preferred Qualifications

• Master’s Degree in Nursing preferred.
