Job Summary The role of the Home Health Nurse 2 is to provide exemplary infusion care to patients in their homes, at our infusion suites, and at bedside in the hospital for discharge education. The Home Health Nurse 2 identifies and implements nursing interventions and evaluates results for a wide variety of patient populations, directs and acts as resource to other members of the nursing team as assigned and promotes compliance and support for policies. The primary responsibilities for this position consist of providing total, comprehensive, continuous and patient centered care which includes: assessing, documenting, planning, implementing, teaching, developing, and evaluating a comprehensive nursing plan of care, delivering direct patient care, ensuring patient satisfaction, training and delegating duties, and adhering to University policies and procedures as well as federal, state and local regulations. Autonomous nursing care is the ability of a nurse to assess and provide nursing actions as appropriate for patient care populations based on competence, professional expertise, and knowledge. The nurse is expected to practice autonomously consistent with ANA Standards of Practice and Professional Performance. Independent judgment is expected to be exercised within the context of interdisciplinary and multidisciplinary approaches to patient care. The Home Health Nurse 2 provides direct patient care to a diversified patient population. Applies knowledge, experience and judgment to determine the importance of a situation, sets priorities, and uses abstract thought to evaluate clinical situations. Participates in patient and staff education. Demonstrates leadership skills. Demonstrates accountability for own practices as defined by the Nurse Practice Act. Apply By Date July 27th, 2026 at 11:59 PM PT Minimum Qualifications - For full consideration, applicants are encouraged to upload license and/or certification if required of the position Valid California Registered Nurse License without restriction American Heart Association (AHA) or American Red Cross Basic Life Support (BLS), or UC Davis Health CPR Required to hold a valid California driver's license, have a driving record that is in accordance with local policies/procedures, and/or enroll in the California Employer Pull Notice Program Graduation from an accredited school of nursing One (1) year of Home Infusion Pharmacy experience One (1) year of experience administering specialty IV medications including biologics Two (2) years of Vascular Access Device experience including accessing, maintaining, and blood sampling Two (2) years of experience placing peripheral IVs and performing peripheral blood draws One (1) year of experience performing bedside teaching to hospital inpatients including self-infusion of acute antibiotic therapies and TPN (teaching should include patient vascular access maintenance) Advanced assessment skills required to recognize early signs and symptoms of an infusion reaction and take necessary steps to prevent patient harm Ability to administer first-dose medications in the home-setting Knowledge and ability to administer anaphylactic medications in an emergency Ability to start peripheral IVs in patients with varying vasculature Ability to admix medications in the home-setting Demonstrated ability in assessment, planning, implementing and evaluating patient care, taking into consideration protective interventions, development (behavioral and physical) and physiological characteristics specific to the patients Possess exceptional verbal and non-verbal communication skills, and present with an open and adaptable demeanor when faced with challenges Ability to read, write, speak, and understand the English language Ability to utilize medical terminology appropriately Knowledge of current trends in nursing Demonstrated commitment to excellence in patient care Effective interpersonal skills Demonstrated accountability and professionalism Ability to adapt to change in a positive, professional and productive manner Knowledge of legal implications in clinical practice and documentation Knowledge of performance improvement and evidence-based practice Basic computer skills Ability to drive distances and shift priorities based on patient care needs Ability to work in an autonomous and professional manner in unique environments Preferred Qualifications Certified Registered Nurse Infusion (CRNI) Vascular Access-Board Certified (VA-BC) Oncology Certified Nurse (OCN) Pediatric Nursing Board Certification (PED-BC) Bachelor of Science in Nursing Inpatient bedside nursing experience Infusion clinic experience Oncology experience Key Responsibilities 80% - Nursing Process and Practice 10% - Quality 5% - Professional Development 5% - Nursing Research Department Overview UC Davis Health is a nationally recognized leader in the provision of high-quality, patient-centered care, based in the Greater Sacramento area of Northern California. The pharmacy services encompass inpatient pharmacy services as well as an expansive footprint in the ambulatory care space where we have clinical pharmacists embedded within our clinics and specialty care locations. Our operational teams support the health system enterprise with three inpatient pharmacies, five outpatient pharmacies, two infusion pharmacies, a home infusion service line, and the full complement of pharmacy purchasing, stores, and distribution.
