In this role, you will partner with our PM&R physician in the sub-acute rehab setting. This position is perfect for those passionate about improving patient outcomes and working within an interdisciplinary, specialized team. About the Role Location: In‑person at skilled nursing facilities. Schedule: Flexible options, Full‑time and Part‑time positions available. Culture: Detail‑oriented, outcome‑focused, stable, people‑oriented, and team‑oriented. Responsibilities Partner with the facility's therapy department to provide the best possible rehab care. Communicate and collaborate with the patient's primary care team to ensure the best outcomes. Perform timely charting/documentation, which can be done from home.
