Requirements:

• Graduationfrom an accredited medical school.
\n
• Completionof an accredited United States Family or Internal Medicine Residency Program.
\n
• BoardCertified or Board Eligible in Family or Internal Medicine.
\n
• ValidArizona medical license.
\n
• ValidDEA certificate.
\n
• Demonstratedunderstanding of clinical methods and techniques.
\n
• Stronginterpersonal skills and the ability to communicate effectively with patients,families, colleagues, and the general public.
\n
• Abilityto provide respectful, culturally responsive care to individuals from diversecultural and socioeconomic backgrounds.
\n
• Basicproficiency in business English, spelling, and punctuation.
\n
• Basiccomputer proficiency, including the ability to use electronic medical recordsystems.
\n
• ValidArizona driver’s license.
\n
• CurrentCPR and AED certification for Healthcare Professionals through coursesfollowing American Heart Association and American Red Cross guidelines,including cognitive and skills evaluations.
\n
• ValidLevel One Fingerprint Clearance Card issued by the Arizona Department of PublicSafety for assignments at specialty behavioral health locations.
