Priority Dispatch
Location: Western & Central OhioSchedule: Monday–Friday, 4:00PM - 12:00AMWeekend Rotation: Every 4th weekend (Friday & Monday off after worked weekend)Holiday Rotation: 2 holidays per yearHours: Full-time (40/week)Summary of Position: The After-Hours Nurse is responsible for direct patient care after business hours.Responsibilities of Position:Provide skilled, compassionate nursing care including symptom management, emotional support, patient/family education, and timely response to needsDeliver care through routine home visits, end-of-life/continuous care, referrals, admissions, care center support, and chart reviewsPerform thorough initial nursing assessments and monitor patient conditionExplain hospice services, insurance benefits (Medicare, Medicaid, private), palliative care interventions, and appropriate use of ABNsCollaborate with physicians and Medical Directors and obtain/enter orders as appropriateOrder and coordinate Durable Medical Equipment (DME) to meet patient needsFacilitate continuity of care, including home-to-inpatient transitions (hospital, nursing home)Complete accurate, timely documentation in compliance with agency, accreditation, and regulatory standardsMaintain clinical competency and participate in weekend rotations or other duties as assignedPay & Benefits:Medical, dental, and vision eligibility 1st of the month following hireEmployer-sponsored direct care program: full-time employees receive free covered medical visits with no deductibles, copays, or out-of-pocket costs (coverage varies by visit type/service)30 paid days off per year (PTO, sick time, and holidays)401(k) eligibility 1st of the month following hireEmployer-paid short-term disabilityTuition reimbursementIncentives and shift differentialsMileage & cell phone reimbursementMonthly employee recognition eventsPayActiv—early access to earned wagesPSLF-qualified employer$200 initial clothing allowance for full-time positionsQualifications:Current Ohio LPN licenseMinimum 2 years LPN experience (Med/Surg, Oncology, or Community Health preferred; hospice a plus)Valid driver’s license and reliable transportationStrong communication and time management skillsAbility to work independently #J-18808-Ljbffr
